Program Manager/Staff Accountant

Looking for a dynamic, energetic, independent team member who can manage accounting duties, office management and programming.

This position is a dual role of Staff Accountant and program management, including administrative and organizational support to the Executive Director for the Southwest Pennsylvania AHEC.  This position is responsible for supporting the overall management practices, including accounting functions and the fiscal and financial process related to federal, state, and local funding agencies within the organization.  The position also includes organizing and assisting with programming events held by Southwest PA AHEC.

 


Pediatric Community Health Worker

Location: East Liberty, Pittsburgh, PA | Annual Salary Range: $37,000 – 42,000

This position is part of a Community Based Care Management Program (CBCM) partnership between UPMC For You (UPMCFY) and Southwest PA Area Health Education Center (AHEC). The Pediatric CHW will be reaching out and meeting with UPMCFY Health Plan members living in the East Liberty and surrounding communities. The CHW will reach out to the household of the child to offer support services, including but not limited to basic needs, social determinants of health navigation, and connecting members/families to primary care providers. CHWs will refer members to appropriate UPMC care management services as needed. CHW will maintain a relationship over time, as appropriate to the needs of the member


Maternity Community Health Worker

Location: McKeesport, PA | Annual Salary Range: $37,000 – 42,000

This position is part of a Community Based Care Management Program (CBCM) partnership between UPMC For You (UPMCFY) and Southwest PA Area Health Education Center (AHEC). The Maternity CHW will be reaching out and meeting with UPMCFY members living in the McKeesport area. The CHW will work with health clinics and local community-based organization to develop a community presence. Through this community presence, the CHW will identify UPMC for You maternity members with a focus on those early in pregnancy and those who are not currently accessing care. The CHW will offer support services, including but not limited to connecting members to basic needs services, social determinants of health navigation, and connecting and coordinating access to perinatal care providers. CHWs will work in collaboration with UPMC Maternity Care Management team to maximize the benefits and access to care for members. CHW will maintain a relationship over time, as appropriate to the needs of the member.
